JOINT AFFIDAVIT TWO DISINTERESTED PERSON FOR

LATE REGISTRATION

We, LORIFE S. MACALALAD, 41 years old, single, and a resident

of Barangay 10 Poblacion, Catanauan, Quezon, Philippines and
REYMART MACALALAD, 21 years old, married, and a resident of
Barangay Madulao, Catanauan, Quezon, Philippines, having been duly
sworn to in accordance with law, hereby depose and state that:

1. We personally know the spouses Mary Jean O. Garay and

Randy R. Garay;

2. We are not related to the Spouses Garay either by

consanguinity or by affinity;

3. We personally know that Mary Jean O. Garay gave birth to

their tenth child in the name of MELAN OMAGA GARAY on
SEPTEMBER 30, 2017 at GATASAN, CATANAUAN, QUEZON,
PHILIPPINES;

4. We personally know the foregoing facts because we were

neighbors for a long time and close family friends. I, Lorife Macalalad was
present when Mary Jean O. Garay gave birth to Melan O. Garay in their
house at Barangay Gatasan, Catanauan, Quezon on 30 September 2017;
and

5. We are executing this Affidavit for the purpose of attesting to

the truth of the foregoing averments and for all other legal purposes this
may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 24 th
