AFFIDAVIT OF DISCREPANCY

I, MARIQUETA REA MALABANA, of legal age, single, Filipino and a resident of

Barangay Tuhian, Catanauan, Quezon, Philippines, after having been duly sworn to in
accordance with law hereby depose and state that:

1. In my Certificate of Live Birth issued by the Philippine Statistics Authority, what

was stated as the maiden name of my mother is her married name: Marietta Rea
Malabana instead of her true maiden name which is: Marietta Reyes Rea as appearing in
Certificate of Live Birth;

2. As a result, there is a discrepancy in the entry of the details in my Certificate of

Live Birth, particularly in the entry of the details of the name of my mother and in the
name of my mother in her Certificate of Live Birth;

3. The name MARIETTA REA MALABANA as entered in my Certificate of Live Birth

and MARIETTA REYES REA as appearing in the Certificate of Live Birth of my mother
refers to one and the same person, and that is my mother; and

4. I am executing this Affidavit in good faith to attest to the truth of the foregoing
facts and for all legal intents and purpose this may serve.
