AFFIDAVIT OF ILLEGITIMACY

I, Maria Ligaya, Filipino, of legal age, single and a resident of 123 Bagong
Liwanag, Barangay Sinilangan, Quezon City, Philippines, having been duly
sworn in accordance with law, hereby depose and state:

1. That I am the biological mother of Mary Anne who was born on February 28,
2021 at the St. Lukes Medical Center, Taguig City, Metro Manila, Philippines;

2. That as appearing in the records of the Office of the Civil Registrar of the City
of Taguig, Philippines, the father of the said child is Mario, also a Filipino
citizen. The same is certified by the attached Birth Certificate of the said child;

3. That I hereby affirm and attest to the truth of the fact that the father of the
aforementioned child is indeed Mario;

4. That I also declare that under the laws of the Philippines, I and Mario were
not incapacitated to marry each other at the time of the conception or birth of
the child, nor are we incapacitated to marry each other now or in the future; and

5. That I am executing this affidavit to attest to the truth of the foregoing facts.
IN WITNESS WHEREOF, I hereby set my hand this 28th day of February 2022,
