AFFIDAVIT OF ADMISSION OF PATERNITY

I, JULIAN ANDREI AVORQUE ACIDRE, of legal age, Filipino, single, and a

resident of Brgy. 81, Marasbaras, Tacloban City, Leyte, after having been duly sworn to
in accordance with law, depose and state that:

1. I am the biological father of the child named “DIONA MAE BRITON” who was
born outside of wedlock at August 4, 2020 at Eastern Visayas Regional Medical
Center;

2. At the time of the conception of my child, I was not married to her biological
mother;

3. I hereby admit under oath the paternity of the child “DIONA MAE BRITON”;

4. She shall henceforth be entitled to all the rights of an acknowledged child/heir

pursuant to Republic Act 9255;

5. I am executing this affidavit to attest to the foregoing facts and for any legal
purpose it may serve.
