AFFIDAVIT OF GUARDIANSHIP

We, Rhanny V. Ostulano and Marjurie K. Ostulano, of both

legal ages, married, Filipino citizens, and presently residing at
527 Celeridad St., Brgy.104, Pasay City, after having been duly
sworn to in accordance with law, do hereby depose and say:

1. That we are the parents and guardian of Leonite Tacdoro Loayon,

a minor, born on;

2. That the above mentioned minor is under my care and custody;

3. That we are competent to receive in behalf of the said minor

any amount/s due him; and

4. That we are not an imbecile, insane, vagrants or vicious persons

or habitual drunkards or habitual criminals, and have not
abandoned, neglected or refused to support said minor or caused
him to commit offenses against the law.

5. That we are executing this affidavit to attest to the truth of all the
foregoing statements and for whatever legal purpose it may
serve.
