AFFIDAVIT OF TEMPORARY GUARDIANSHIP

I, KAREN JOY CORPUZ CASTRO, Filipino, of legal age, single

and a resident of Talogtog Sur, San Juan, La Union, after
having been duly sworn to in accordance with law, do hereby
depose and state:

1. That I am the biological mother of the minor child,

XIMON MATTHIEU CASTRO NAVOS, male, 3 years old and born
on March 6, 2015;

(A copy of Ximon Matthieu Castro Navos’Certificate of

Live Birth is herein attached as Annex “A”).

2. That I and the father of XIMON MATTHIEU are not married

to each other, although the father, Patrick Alba Navos
is already married to another woman;

3. That XIMON MATTHIEU now lives with me at Talogtog Sur,

San Juan, La Union;

4. That I am left with the sole responsibility of raising

my child without any help whatsoever from his
biological father;

5. That I am a nurse by profession and will be leaving

soon for Dammam, Saudi Arabia to work as a Nurse;

6. That in my absence I would like to appoint a temporary

guardian to my child to provide the necessary care,
love, affection and guidance while I will provide for
the financial means and needs of my child from my work
in Dammam, Saudi Arabia; and

7. That the need for temporary guardianship arose due to

my desire to work overseas and to earn and prepare for
my child’s future while at the same time provide for
his proper care, ensure his nutrition, health, shelter,
and physical safety while I am away.

 Anent the foregoing, I hereby appoint and duly authorize
my parents, NAPOLEON A. CASTRO (father) and SALLY CORPUZ
CASTRO (mother), both Filipinos, of legal age and residents
of Talogtog Sur, San Juan, La Union to exercise concurrently
guardianship of my child, XIMON MATTHIEU CASTRO CORPUZ and
any and all rights and responsibilities relative thereto, and
to perform any and all acts deemed necessary and appropriate
for a temporary Guardian of a minor child including the
following but not limited to: Health and Medical
care,Education and Travel.

(An Affidavit of Undertaking by the appointed temporary

guardians are herein attached as Annex “B”).

Relative to this appointment and authority, the temporary

Guardians are NOT authorized to do or perform the following:

a) Transfer residence of the child without consent from

the mother;

b) Allow any other person or persons other than herein

guardians to bring the minor child outside their
residence without their knowledge and permission;

c) Allow the biological father of the child to take him

outside the premises of their residence without the
written consent of the mother;

d) Make a decision that conflicts with the decision of the

minor child’s mother;

e) Entrust the care of the minor child to any other person

or persons other than themselves for whatever reason;
and

f) Such other acts that may be prejudicial and detrimental

to, but not limited to the physical, emotional and
mental development of the minor child.

Moreover, I hereby declare and certify that there are no

court orders currently in effect that would prohibit me from
exercising or conferring the aforementioned rights and
authority upon the herein named Temporary Guardians.

I further declare that I freely and knowingly confer the

above mentioned rights and responsibilities in order to

provide for the minor child and not as a result of pressure,
threats or payment by any person or agency.

In the event that I wish to amend or revoke this

affidavit, I will provide a copy of the amended affidavit or
revocation to all parties to whom I provided a copy of this
original affidavit.

This Temporary Guardianship shall become effective on this

date and shall remain in full force and effect until such
time the appointing party or the affiant, shall notify the
designated temporary guardians in writing that this document
has been amended or revoked.

BE IT KNOWN, under the penalty of perjury, hereby affirm

that the above statements are true and correct to the best of
my knowledge.

IN WITNESS WHEREOF, I have hereunto affixed my signature
