AFFIDAVIT OF NON-FILING OF INCOME TAX

I, FRANK A. TAGUITAG, of legal age, Filipino, married, and resident of and with
postal address at 202 Foothill St., Crystal Cave, Bakakeng Central, Baguio City after
being duly sworn to in accordance with law hereby depose and state that:

I am one of the heirs of BENJAMIN S. ETILONG; that he is the owner of a parcel

of land situated in San Fermin, Caba, La Union with an area of 26,493 square meters,
more or less and covered by OCT NO. CLOA -2901 (TD NO. 2009-09-0010-00078);

Jamie’s parents, Spouses Joel M. Borja and Jeniffer C. Borja, both OFW and
presently working as Civil Engineers in Perth, Australia have no source/s of income
within the Philippines;

I am executing this Affidavit in behalf of JAMIE C. BORJA anent the issuance of a

Certification of Non-Tax Payment/Non-Filing of ITR as required by the Bureau of
Internal Revenue (BIR) relative to the requirements for EducatIon Service
Contracting (ESC) application for JAMIE C. BORJA;

I attest to the truthulness and veracty of the foregoing.
