JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS
We, EVANGELINE C. MORENO, of legal age, Filipino citizen,
widower, and a resident of Barangay 2nd, Camiling, Tarlac and BILLY
B. AQUINO, of legal age, Filipino citizen, married and a resident of
Barangay Pao 1st, Camiling, Tarlac, after having been duly sworn to in
accordance with law, do hereby depose and state THAT:

1. We both personally know JIMMY SANGALANG PALASIGUE

(Jimmy, for brevity), who was born on February 3, 1968 to
Spouses Rudy G. Palasigue and and Sixta V. Sangalang in
Camiling, Tarlac;

2. Jimmy is a member of the Social Security System of the

Philippines (SSS) until his death on January 3, 2017;

3. When Etherlina Magaoay Palasigue, the wife of Jimmy, went to

the SSS Camiling, Tarlac to file for the Death Claim Benefit of
her late husband, she was required to submit, among others,
records of the birth, marriage and death of Jimmy;

4. Upon submission of the required documents, it was discovered in

the Certificate of Live Birth of Jimmy that the recorded Name of
his mother is SIXTA V. SANGGALANG, instead of SIXTA V.
SANGALANG as recorded in his Marriage Contract and Certificate
of Death. Hence, the discrepancy;

5. We are not related to Jimmy either by consanguinity or affinity,

except that of being his friends and acquaintances;

6. We are executing this affidavit to attest to the fact that the

names SIXTA V. SANGGALANG, appearing as the mother of
Jimmy S. Palasigue in his Certificate of Live Birth, SIXTA
SANGALANG, appearing as the mother of Jimmy Palasigue or
Jimmy Sangalang Palasigue in his Marriage Contract and
Certificate of Death, refer to one and the same person, and that
is the mother of our friend and acquaintance, now deceased;
 7. Finally, we are executing this Affidavit to attest to the truth of
the foregoing statements and for all legal intents and purposes it
may serve.

IN WITNESS WHEREOF, we have hereunto affixed our
