AFFIDAVIT OF CORRECTION

I, Joyce Olsim Malute-Galino, of legal age, married, Filipino, and resident of 51 A,

Happy Homes, Old Lucban, Baguio City, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and state THAT:

1. I am the mother of Israel Malute Galino (Israel), who was born on January 18, 2005
at Benguet General Hospital, La Trinidad, Benguet;

2. Recently, when I secured a copy of the Certificate of Live Birth of my son, Israel,
with the Philippine Statistic Authority (PSA) for personal consumption, I was
surprised to discover the error or mistake therein, specifically the entry on the date of
marriage of parents, which was indicated as July 14, 2004;

3. The said date of marriage of parents is an erroneous entry considering that the true
and real date of our marriage is July 19, 2004 and not July 14, 2004. Attached hereto
is a copy of the Certificate of Marriage marked as Annex “A”;

4. In the meantime there is an apparent but innocuous error/mistake in my son Israel’s

Certificate of Live Birth as mentioned above;

5. The commission of the aforementioned error was due to an excusable and honest
mistake;

6. This Affidavit is being executed in order to set the records of birth of my son Israel
straight as well as to avoid confusion over his identity. Finally, I am executing this
affidavit to attest to the truthfulness of the foregoing facts and for all legal intents and
purposes it may serve well.

IN WITNESS WHEREOF, I have hereunto set my hand this 23rd day of March 2017 at
