PETITIONER, unto this Honorable Office of the Registry of Deeds for

the Province of Lanao del Norte, most respectfully states that:

1. Petitioner _________ is of legal age, Filipino, and a resident of

Poblacion, Kapatagan, Lanao del Norte.

2. Petitioner is the owner of a parcel of land, designated as Lot 43-E-9

Psd-10-064111, with remaining area of TWO THOUSAND EIGHT
HUNDRED TWO SQUARE METERS (2,802 sqm.), situated in Poblacion,
Kapatagan, Lanao del Norte, covered by Transfer Certificate of Title (TCT)
No. T-28,424 (a.f.) of the Registry of Deeds for the Province of Lanao del
Norte.

3. Petitioner donated two (2) portions of Lot 43-E-9 Psd-10-064111 to

his granddaughter ____________________, which portions are identified as:
Lot 43-E-9-C, Psd-10-073505, containing an area of Five Hundred Square
Meters (500 sqm.); and, Lot 43-E-9-D, Psd-10-073505, containing an area of
Five Hundred Square Meters (500 sqm.). Copies of the technical descriptions
of Lot 43-E-9-C and Lot 43-E-9-D are hereto attached and marked as Annex
“A” and Annex “B,” respectively.

4. The remaining portion of the said parcel of land identified as Lot 43-
E-9-E, Psd-10-073505, being a portion of Lot 43-E-9, PSD-10-064111, contains
an area of ONE THOUSAND EIGHT HUNDRED TWO SQUARE METERS
(1,802 sqm.), more or less. A copy of the technical description of Lot 43-E-9-E,
Psd-10-073505 is hereto attached as Annex “C.”

5. The above-described technical descriptions and plans had already

been approved by the Assistant Regional Director for Technical Services,

Region X, Cagayan de Oro City, AGNES A. DEJORAS, OIC, on 14 March
2017.

6. Petitioner is now desirous of having the donated portion to

_________________ be separated from TCT-28,424 (a.f), and, consequently, for
a TCT to be issued over Lot 43-E-9-C, Psd-10-073505, covering 500 sqm., and
Lot 43-E-9-D, covering 500 sqm., separately from TCT-28,424 (a.f).

7. Furthermore, Petitioner likewise prays for a separate title to be

issued over the remaining portion identified as Lot 43-E9-E, covering 1,802
sqm. in his name, and for the same to be separated from TCT No. T-28,424
(a.f.).

PRAYER

WHEREFORE, petitioner respectfully prays of the Honorable Registry

of Deeds for the Province of Lanao del Norte to ISSUE a separate title over Lot
43-E-9-C, Psd-10-073505 and Lot 43-E-9-D, Psd-10-073505 in the name of the
DONEE, ___________, as the new owner thereof, after legal fees be paid; to
ISSUE another separate title over Lot 42-E-9-E, Psd-10-073505 in the name of
herein petitioner, ___________, as the new owner thereof, after legal fees be
paid.

Petitioner likewise prays for other relief or remedies as may be

applicable under the premises.
