I, MARILOU G. ACLON, of legal age, Filipino, married, and a

resident of New Capitol Site, Tagbilaran City, Bohol, Philippines, after having been
duly sworn in accordance with law, hereby depose and state, that:

1. I am the transporter of the following items listed below;

2. That the items listed are certified true and correct:

a. 69 pcs. of lumber/timber/etc. with a volume of 1.01 cu./430.8 board
feet of Mahogany and Gemelina.

3. That the same are to be transported from Moto Sur, Loon, Bohol to
Taloto, Tagbilaran City on August 13-14, 2019;

4. That I am the authorized claimant of the listed items above and it is

for personal usage/consumption only and not for commercial purposes;

5. I am executing this affidavit to attest to the truth of the foregoing and

for any legal purpose that this may serve.
