AFFIDAVIT OF CONSOLIDATION

I, (name and personal circumstances of Seller), after having duly sworn, depose and say:
1. On ________20 ___ I entered into a Deed of Sale with Right to Repurchase (Pacto de
Retro Sale) of a parcel of land located at ____________ and covered by Transfer Certificate of Title
No. __________ of the Register of Deeds of ________ including the existing improvements thereon,
with (name of Seller), which was acknowledged before Notary Public Atty. (name of Notary Public),
and entered in his Notarial Register as Doc. No. Page No._____; Book No.____; Series of
______; a copy of which is attached as Annex "A".

2. Under the said Deed of Sale, the said Seller ___________ was given a period of _____
months/year from the execution thereof, or until ____________ within which to repurchase said
property. However, the period to repurchase said property had already expired, but the Seller did not
exercise his right to repurchase the same. The period to repurchase was not expressly or impliedly
extended.
3. For failure of the Seller to exercise his option to repurchase, the absolute ownership of
the said property is now consolidated in my as it has already been approved and confirmed in the
Order dated _________ issued by (court) in Civil Case No. ___________ a certified copy of which is
attached as Annex "B".
4. I have executed the foregoing to attest to the truth of the above averments; and to
request the Register of Deeds of ____________ to effect and record the consolidation of the title of
the subject property in my name.
