sworn, depose and say, that:
1. We personally know the Spouses Juan Dela Cruz and Maria Dela Cruz because
they are our close friends and our classmates in college.

as Maria Juana Dela Cruz.
3. We know this fact as we are close friends and neighbors when their said first
child was born. We were among those special guests of our said friends during the baptism
of their said child; and we sometimes attended to the said child's school activities whenever
our friends are at their work.
4. We have executed this affidavit to attest. to the fact that "MA. JUANA DELA
CRUZ" and "MARIA JUANA DELA CRUZ" refer to one and the same person, the first
child of our friends, Spouses Juan Dela Cruz and Maria Dela Cruz; and for all legal intents
and purposes.
