AFFIDAVIT OF LOSS

I, (name and personal circumstances of the affiant), and a resident of_____________, after
having duly sworn, depose and say, that:
1. I am the registered owner of a parcel of land located at _______, containing an area
of________ square meters, more or less, described and covered by Transfer Certificate of Title No.
_________of the Register of Deeds of ________.
2. I used to keep my Owner's Copy of the said title in my drawer at my aforesaid
residence. Just recently, I discovered that the said Owner's copy was missing from the said drawer;
and thinking that it was only misplaced, I exerted efforts to locate its whereabouts, but to no avail.
Hence, I have considered the same to have been lost and beyond recovery.
3. I did not sell, mortgage or in any manner dispose of my said property to whomsoever;
and that there is no pending transaction involving said property for registration with the said
Register of Deeds.
4. I have executed this affidavit to attest to the truth hereof; and for purposes offiling the
necessary petition for issuance of second owner's copy of TCT No.________; and for all legal
intents and purposes.
