ATTESTATION CLAUSE
WE, the undersigned attesting witnesses, do hereby certify'
that the Testator (name-of-Testator), whose signature appears at

the same person who declared to all of us that the foregoing Last
Will and Testament, which we signed as witnesses, was his/her
Last Will and Testament. Said Testator signed this Will at the
