DEED OF REAL ESTATE MORTGAGE

KNOW ALL MEN BY THESE PRESENTS:

This MORTGAGE is made and executed this __th day of _______
at __________by and between:

referred to as MORTGAGOR;

(Name and personal circumstances of Mortgagee); and hereinafter
referred to as MORTGAGEE.

WITNESS ETH:
The MORTGAGEE extended to and the MORTGAGOR obtained a
cash loan in the amount of _________(Php ______), which is embodied
in the Promissory Note dated ________attached as Annex "A" hereof.
As security for the said loan, the MORTGAGOR hereby conveys
and transfers by way of mortgage unto and in favor of the
MORTGAGEE, his parcel of land situated at __________and covered by
Transfer Certificate of Title No. ________and more particularly described
as follows:

under the following terms and conditions.
1. All expenses, charges taxes and assessments in connection
with this mortgage shall be shouldered by the MORTGAGOR.
2. Should the MORTGAGOR, his heirs, assigns, or
administrators shall fully comply and settle the subject indebtedness and
performed his obligations under the terms and conditions set forth herein,
then this mortgage shall become null and void; otherwise, it shall be in
full force and effect and enforceable in the manner provided by law.
