JOINT-AFFIDAVIT OF LEGITIMATION
WE, SPOUSES JOEY TOM DURANT and EDNA ALMOVAR MIRANDA,
both of legal age, Filipino citizens and residents of Brgy. Leones East, Tubao, La Union,
after first being duly sworn to on oath in accordance with law, hereby depose and state
that:

That we are the biological parents of Jaime Miranda Durant who was born on
April 19, 2012 at Agoo Maternity and Lying-in Clinic;

That at the time of birth of Jaime Miranda Durant, we were not yet married but we
have been co-habiting as husband and wife since 2005.

, That subsequently, we got married on December 30, 2013 at the Saint Isidore
Parish, Tubao, La Union, as evidenced by our Certificate of Marriage attached hereto as
Annex “A”;

That we are executing this Joint-Affidavit to have our son, Jaime Miranda Durant
legitimated, to attest to the truthfulness of the above-narrated facts, and for all legal
intents and purpose
