JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, PEDRO LYNDON and BRYAN JORGIO CADILIG both of legal age,
Filipino, and both residents of Lower Batinguel, Dumaguete City, Negros Oriental,
after having been sworn to in accordance with law, hereby depose and state that:
1. We personally know the spouses VERGENCITA DY TANILON and SAMUEL
TOQUERO TANILON, Filipinos, of legal age and resident of 482 Lower
Batinguel, Dumaguete City, being his neighbors and close friends;
2. VERGENCITA DY TANILON died on August 2, 2020 in Dumaguete City;
3. Unfortunately, in some of the official document of VERGENCITA DY
TANILON, specifically in Marriage Contract, SAMUEL TANILON and
DUCHESNE TANILON-LAURENTE, and in the Birth Certificate of Adrian
TANILON, her name appeared as VIRGENCITA DY TANILON. Further in
the Marriage Contract of SAMUEL TANILON JR., her name appeared as
VIRGINIA DY, while in the Marriage Contract of DEXTER TANILON, her
name appeared as VIRGINCITA DY;
4. We hereby state under oath that the names VERGENCITA DY TANILON
appearing on the Records of the DCCCO and in her Death Certificate, and
the name erroneously entered as VIRGENCITA, VIRGINIA and
VIRGINCITA, appearing on the documents mentioned in the preceding
paragraph, all refer to one and the same person;
5. Further, the name SAMUEL TOQUERO TANILON as appearing in the
records of the DCCCO and the name SAMUEL TOQUIRO TANILON as
appearing on the Passport ID and Marriage Contract of SAMUEL TANILON,
refers to one and the same person. In addition, in the Birth Certificate
ADRIAN TANILON and Marriage Contract of DUCHESNE TANILON-
LAURENTE, the name of SAMUEL TOQUERO TANILON was entered as
SAMUEL TOQUERO TANILON SR., and that both names refer to one and
the same person as well;
6. Also, in the Date of Birth of DEXTER D. TANILON, the records of the
DCCCO shows January 6, 1985, however, his true Date of Birth is January
7, 1985 as appearing in his Unified Multi-Purpose ID (UMID) and in his
Marriage Contract. We know this for a fact as we also personally know
DEXTER TANILON to be the son of spouses VERGENCITA DY TANILON
and SAMUEL TOQUERO TANILON;
7. We are not related by consanguinity or affinity to spouses VERGENCITA
DY TANILON and SAMUEL TOQUERO TANILON, and to their son DEXTER
TANILON;
8. We are executing this affidavit to attest to the truth and veracity of the
foregoing facts and for whatever legal and beneficial purposes this may
serve.
