I, FREDILYN MIGUEL BALTAZAR, of legal age, Filipino, single,

and a resident of Tupax, Jones, Isabela, after having been sworn in
accordance with law, hereby depose and state that:

1. I am the child of RESTITUTO BALTAZAR and LIBERATA

MIGUEL;

2. I am known by the name “FREDELYN MIGUEL BALTAZAR”

and have used the same in my personal and official records, as
shown by the following (a copy of which is hereto attached as
Annexes “A” and “B”);

-Voter’s ID

-Philhealth Card

3. My name was recorded on the following as “FREDILYN

MIGUEL BALTAZAR” a copy of which is hereto attached as
Annex “C”;

- Certificate of Live Birth registered in the Civil Registry of Jones

4. Both the names “FREDELYN MIGUEL BALTAZAR” and

“FREDILYN MIGUEL BALTAZAR” refer to one and the same
person, the Affiant;

5. I am executing this affidavit to attest to the truthfulness and

honesty of the foregoing facts and for all intents and purposes it
may serve.

Jones, Isabela, after having been duly sworn to in accordance with law, do
hereby depose and state:

1. That we personally know FREDILYN MIGUEL BALTAZAR

whose parents are SPS. RESTITUTO BALTAZAR and
LIBERATA MIGUEL;

2. That she was born on 23 August 1966 at Tupax, Jones, Isabela;

3. That we know her to be using as her name FREDELYN

MIGUEL BALTAZAR in all of her private and public dealings
instead of FREDILYN MIGUEL BALTAZAR her true and
correct name appearing on her Certificate of Live Birth;

4. That we are executing this affidavit to attest into the truth of the
fact that the true and correct first name of FREDILYN MIGUEL
BALTAZAR is FREDILYN and this fact is known to us
because we are his close friends;

5. That FREDILYN MIGUEL BALTAZAR and FREDELYN

MIGUEL BALTAZAR are one and the same person.
