JOINT AFFIDAVIT OF DISINTERESTED PERSONS

We, MADELYN F. BERNARDO and ESMENIA L. BERNARDO, both of legal age, Filipino
citizens and residents of Brgy. 23, Santa Cecilia, San Nicolas, Ilocos Norte, Philippines, after
having duly sworn to in accordance with law do hereby depose and state that:

1. We personally and intimately know MIRIAM RUTH B. ASUNCION a.k.a

MIRIAM B. ASUNCION because we all live in the same barangay, town and
province;

2. We know as a fact that the name MIRIAM RUTH B. ASUNCION was the registered
name at the Office of the Municipal Civil Registrar, San Nicolas Ilocos Norte as
well as in the Certificate of Live Birth issued by the Philippine Statistics
Authority (PSA) while in most of her pertinent records and dealings whether
public or private, she was and has been using the name MIRIAM B. ASUNCION
not MIRIAM RUTH B. ASUNCION;

3. The above described discrepancies are all unintentional and without the
intention to defraud anyone or conceal the true identity of MIRIAM RUTH B.
ASUNCION A.K.A MIRIAM B. ASUNCION;

4. We are hereby executing this affidavit to attest to the truth that the names
“MIRIAM RUTH B. SANTIAGO” and “MIRIAM B. SANTIAGO” refers to one and the
same person;

5. Finally, we are executing this affidavit for all legal intents and purposes this may
be worth.
