AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, BASILISCO B. ELIMANCO Jr. and VICTOR M. ABUCAY, both of legal age, Filipinos, and
residents of Purok 2 Avocado St., Tablon, Cagayan de Oro City and Johndorf Subd., Barra, Opol,
Misamis Oriental, after having been sworn to in accordance with law hereby depose and say;

1. That we personally know RUDIBALDO ORITO SABALO of Brgy. San Jose, Maasin, Southern
Leyte;

2. That his name as appearing in the Death Certificate of his son (NOEL E. SABALO) is RODOLFO
O. SABALO and in the Marriage Contract of his son and daughter in law (NOEL E. SABALO and
MARIA DONNA G. SABALO) is RUDY O. SABALO, whereas in his records with Senior Citizen,
his name appeared as RUDIBALDO O. SABALO;

3. That the reason why his first name are spelled as RODOLFO O. SABALO and RUDY O. SABALO
instead of RUDIBALDO O. SABALO in his son’s Death Certificate (NOEL E. SABALO) and
Marriage Contract was due confusion of the person who prepared the document during the
process in ascertaining the real spelling of his first name;

4. That in all his documents, he used the name RUDIBALDO O. SABALO, as it is the correct
spelling of his name;

5. That RODOLFO O. SABALO in his son’s Death Certificate (NOEL E. SABALO) and RUDY O.
SABALO in the Marriage Contract of his son and daughter in law (NOEL E. SABALO and MARIA
DONNA G. SABALO), and RUDIBALDO O. SABALO in his records in Senior Citizen refer to one,
single and the same person and there is no other person in our locality with the same name as
that of his.;

6. That we know the foregoing facts because we are long time friends of RUDIBALDO O.
SABALO;

7. That we are not related to him either by consanguinity or affinity and has no interest in
whatever this affidavit could be of use either directly or indirectly;

8. That we execute this affidavit to attest the truthfulness of the foregoing facts and establish
that RODOLFO O. SABALO, RUDY O. SABALO and RUDIBALDO O. SABALO refer to one, single
and the same person, and for whatever legal purpose this affidavit may serve.
