AFFIDAVIT OF HEIRSHIP

WE, Father Name and Mother name, of legal age, Filipinos, and residents of
your Address, after being duly sworn to in accordance with law, depose and state
THAT:

1. We are the parents and the LEGAL SURVIVING HEIRS of RICHIE

VILLANUEVA ALAMEDA, who died on 6 April 2020 at 17A High Street
Egham, Surrey, United Kingdom.

2. A photocopy of a death certificate is attached hereto as evidence of the death

of our son, RICHIE VILLANUEVA ALAMEDA.

3. As evidence of our relation to the deceased, a photocopy of the birth

certificate of RICHIE VILLANUEVA ALAMEDA is also attached hereto,
indicating our names, Father Name and Mother name, as the father and mother
respectively of the deceased.

4. Our son does not have any children or any other compulsory heir under the
law other than us.

5. We are executing this affidavit in good faith to attest to the truth that there is
no other heir or heirs except for the above-named heirs of the said deceased, and
for whatever legal purpose it may serve.
