AFFIDAVIT OF DISCREPANCY
(RE: ONE AND THE SAME PERSON)

I, RAMONITO P. PINEDA, of legal age, married, Filipino, and a resident of Balas, Mexico, Pampanga, after
having been sworn in accordance with the law, do hereby depose and say:

1. That I was born on March 22, 1965 from Spouses Feliciano S. Pineda and Teresita B. Paguio at Balas,
Mexico, Pampanga.

2. That since I came to reason continuously up to present, I was made to believe and understand and in fact have
been using the name “RAMONITO” in all my records;

3. That several document concerning my identity such as but not limited to my Driver’s License, Annex “A”
hereof, Brgy. Kagawad ID, Annex “B” hereof, my marriage contract, Annex “C” hereof, my son’s birth
certificate as his father, Annex “D” hereof, and my daughter’s birth certificate as her father, Annex “E”
hereof, bear and carry the First name “RAMONITO” ;

4. That however, when I secured my birth certificate from the Philippine Statistics Authority (PSA) Office. I
found out that my name was inadvertently written as RAMONCITO PINEDA instead of “ RAMONITO
PINEDA”, which is my true and correct name. Attached herewith the copy of my birth certificate marked as
Annex “ F”;

5. That the same is now presumed to be an honest mistake of the receiving clerk or the informant and was made
in good faith without the intention of defrauding any third person;

6. That the name “RAMONITO” appearing on “Annex A,B,C,D, and E and “RAMONCITO” appearing on
Annex “F” belong to one and the same person, the undersign herein;

7. To remove confusion as to the two names, I hereby declare that my true and correct name is “RAMONITO”
and that my real and full name is “RAMONITO PAGUIO PINEDA”;

8. That the discrepancy appearing on the said Birth Certificate was merely due to clerical error and possibly
committed due to inadvertences of the person who caused the preparation thereof;

9. That I am executing this affidavit to attest the truth of the foregoing statements and to establish my real
identity and for all legal intents and purposes it may serve.

(RE: ONE AND THE SAME PERSON)
We, SALVADOR M. TEODORO and ALEXANDER M. BASILIO, both of legal age, Filipinos, and
residents of Balas, Mexico, Pampanga, after having been sworn in accordance with the law, do hereby depose and say:

1. That we personally know RAMONITO P. PINEDA, who is our long time neighbor and acquaintance at
Balas, Mexico, Pampanga;

2. That there is a discrepancy in his Birth Certificate in which his first name is written as “RAMONCITO”
instead of “RAMONITO” (without letter “C”). Attached herewith as Annex “A”;

3. That several document concerning his identity such as but not limited to his Driver’s License, Annex “B”
hereof, Brgy. Kagawad ID, Annex “C” hereof, his marriage contract, Annex “D” hereof, his son’s birth
certificate as his father, Annex “E” hereof, and his daughter’s birth certificate as her father, Annex “F”
hereof, bear and carry the First name “RAMONITO”;

4. That we know for a fact that the person by the name of “RAMONCITO” as appearing in his birth certificate
and the name “RAMONITO” as appearing in his several documents, refer to one and the same person;

5. That we also known him as the legal husband of LUCIA SESE PANLAQUI, his wife;

6. To remove confusion as to the two names, we hereby declare that his true and correct first name is
“RAMONITO” and that his real and full name is “RAMONITO PAGUIO PINEDA”;

7. That the discrepancy appearing on the said birth certificate was merely due to clerical error and possibly
committed due to inadvertences of the person who caused the preparation thereof;

8. We are not in any way related to any of the foregoing person within the fourth civil degree either by
consanguinity or affinity, nor do we have any pecuniary interest in this matter whatsoever;

9. That we are executing this affidavit to attest the truth of the foregoing statements and to establish the real
identity of RAMONITO P. PINEDA and for all legal intents and purposes it may serve.
