AFFIDAVIT OF TWO DISINTERESTED PERSON

We, BONIFACIA A. ORNOPIA AND APOLINARIA C. MONROID , of legal

age, Filipino,(single / married / widow), and a resident of San Miguel st.
Marteres Cebu City , Philippines, having been duly sworn in accordance
with law, hereby depose and state:

That we personally know PETRA AMACA the wife of Estelito Flores residence
of Jolomaynon , Dalaguete Cebu.

That due to sheer inadvertence ,her name was entered erroneously in

their certificate of marriage as PETRA AMAYA instead of her true name as
PETRA AMACA,

That PETRA AMAYA AND PETRA AMACA were refer to one and of the
same person.

That we know these facts being a close family friend for a long period of
time.

That we are not relatives either by blood or by any consanguinity.

That we are executing this affidavit to attest to the truth of the

foregoing facts and to whatever legal purpose it may serve.
