AFFIDAVIT OF TWO (2) DISINTERESTED PERSONS and

ONENESS OF NAME

WE, ENREQUITA P. RODRIGUEZ and LYDIA BESARIO, both of legal age, married,
Filipinos and residents of Pacuan, La Libertad, Negros Oriental, after having been duly sworn to
in accordance with law, do hereby depose and state, THAT:

1. We both personally know PELISITO GIMENA BARGASA, him being our friend, but are
not related to him by consanguinity or affinity;

2. We also know for a fact that he was using his full name as PELISITO GIMENA
BARGASA, as shown in a certification issued by the Philippine Statistics Authority,
Office of the Civil Registrar General and can be verified from the Local Civil Registry
Office of La Libertad, Negros Oriental, in all of his documents and transactions;

3. Unfortunately, the following typographical errors were made in a few of his official
documents, as follows:

 PILISITO HIMINA BARGASA - Baptismal Certificate and SSS

 FELECITO GEMINA BARGASA - Marriage Certificate

4. We hereby state, under oath, that the names PILISITO HIMINA BARGASA (as reflected
in his Baptismal Certificate and SSS) and PILISITO HIMINA BARGASA (as reflected in
his Marriage Certificate) refer to one and the same person;

5. We are executing this Affidavit upon the request of Evangeline L. Bargasa;

6. We further execute this Affidavit to attest to the truth of the foregoing and for whatever
legal intents and purposes this may serve best.
