JOINT AFFIDAVIT OF LEGITIMATION

WE, FLORINTINO JR. S. DOROSAN and SALLY CUERDO, both of legal age,
husband and wife, respectively, and presently residing at Bagong Sirang Pili,
Camarines Sur after having been duly sworn to in accordance with law, depose
and state:
1. That we are the biological parents of Haide Cuerdo who was born on
October 8, 2001 in_______ as evidenced by the attached copy of the child’s Birth
Certificate, marked as Annex “A”;

2. That at the time of his/her birth, we, the biological parents were not yet
married;

3. That at the time of the conception of our child, there was no legal
impediment for us to contract marriage;

4. That we were married on 30th of January 2002 in __________________,

as evidenced by the attached Marriage Certificate, marked as Annex “B”;

5. That by virtue of our subsequent marriage, the said child is now

legitimated by operation of law; and

6. That we are executing this Affidavit to attest to the foregoing facts, for
purposes of complying with the requirements in applying for the legitimation by
subsequent marriage of our son/daughter, _____________________ and for all
legal intents and purposes.

AFFIDAVIT OF PATERNITY AND USE OF FATHER’S SURNAME

I, FLORENTINO JR. STA ROSA DOROSAN of legal age, filipino citizen, and
presently residing at Bagong Sirang Pili, Camarines Sur, after having been duly
sworn to in accordance with law, do hereby depose and state:

1. That I am the biological father of HAIDE CUERO;

2. That my child HAIDE CUERO was born on October 8, 2001 at Bagong

Sirang Pili, Camarines Sur;

3. That at the time of the birth of my child, I was not legally married to her
biological mother

4. That being the father of HAIDE CUERDO, I hereby acknowledge and

recognize ___________________________________ as my child and legal heir;

5. That I am allowing my child HAIDE CUERDO DOROSAN to use my

surname to be her surname in her Certificate of Live Birth; and

6. That I execute this affidavit to attest to the truth of the above facts in
compliance with the provisions of Republic Act No. 9255.
