AFFIDAVIT of DECLARATION OF GUARDIANSHIP

I, JAY LUCIO F. AGAD, Filipino, of legal age, married and a resident

of Pleasant Homes Subdivision, Punta Princesa, Cebu City after having been
sworn to on oath in accordance with law, depose and say:

1. That I have in my custody JOMAR REALES who was born on

December 10, 1999 in Valenzuela City;

2. That he has been under my custody because his mother Jocelyn

Reales left him while his father Inoel Guevarra’s whereabouts is
unknown;

3. That he is dependent on me for financial, emotional and

psychological support;

4. That I undertake to take and declare custody of JOMAR REALES

as his guardian due to the incapacity/absence his parents;

5. That this affidavit is executed to attest and confirm the truth of the
foregoing statement and for whatever purpose this may serve.
