AFFIDAVIT OF DENIAL

I, ENGELFREDO D. CAGIGAS, SR., of legal age, Filipino, married, and a resident of

147 Katipunan St., Labangon, Cebu City after having been sworn to in accordance with law do
hereby depose and say THAT:

1. I am a bonafide member of the Social Security System with SSS Number 06-1196664-3;

2. I requested for an employment history record from the above-mentioned agency;

3. Upon examination of the said record, it was found that erroneous entries were recorded;

4. Said entries made it appear that I was previously employed a BENITO MANABAT;

5. The said entry is not true considering that I have never been employed with the above-
mentioned employer;

6. I categorically deny having been employed with Benito manabat and I request that the
said employer be deleted from my employment history record;

7. I am executing this affidavit to attest to the fact that I was not employed by the said
employers, and for whatever legal purpose it may serve.
