AFFIDAVIT

I, LEE R. ELTAGONDE, of legal age, married, Filipino citizen and a

resident of Purok Saging, Paligue, Hagonoy, Davao del Sur, Philippines, upon
duly sworn to in accordance with law do hereby depose and state, THAT:

1. I am the same and identical person who caused the execution of this
instrument;

2. I am the registered owner of a MENLEE STORE located at Paligue,

Hagonoy, Davao del Sur;

3. Since the establishment of my sari-sari store sometime in September

2019 and until now I have no employee as my wife and I personally
manage our store;

4. I am executing this Affidavit to attest to the truth and veracity of the

foregoing statements and for whatever legal purpose this may serve.
