AFFIDAVIT OF NO DEDUCTION

I, LEA C. AYOP, of legal age, Filipino, married and resident of

Rizal Ave. Barangay San Jose, Digos City, Davao del Sur, after having
been duly sworn to in accordance with law, hereby depose and say, that:

1. I am the proprietor of a certain business named CLA Glass and

Aluminum Supply;

2. The operation started last January 18, 2010 up to present;

3. That said business hired three (3) employees;

4. That no deduction for Pag-ibig contribution was made during the

said operation in their compensation;

. I am executing this Affidavit to Pag-Ibig Fund of the truthfulness of
the foregoing and for any legal purposes that it may serve.
