AFFIDAVIT OF INCOME

I, JAMES PATRICK ANCHETA MANEGDEG, of legal age, Filipino Citizen with

residence address at No. 49 Truman St. Parkwood Green Market Ave., Pasig City, after
being duly sworn to in accordance with law, depose and state: That:

1. That I am applying for Short Term Loan Home Development Mutual Fund also
known as Pag-ibig;

2. As such I was required to declare my monthly income;

3. That I am earning Fifteen Thousand Pesos (Php15,000.00) as a Family Driver to

Rene Florendo JR II.

4. Through this Affidavit I hereby declare under oath that I am earning Fifteen
Thousand Pesos (Php15,000.00) a month, which I derived from my above-
mentioned work;

5. That I am executing this Affidavit to attest to the truth and veracity of the
foregoing and for whatever legal purpose the same may serve.
