JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, CORAZON V. REYES, of legal age, Filipino citizen, and with address at 19-A
Tuna St., Caloocan City, and MARY ANN D. ELTANAL, likewise of legal age, Filipino citizen,
and a resident of Mayor Angeles St. San Roque, San Roque, after having been duly sworn to in
accordance with law, do hereby depose and state that:

1. The we personally know MARI CURIE B. ONTOG born on June 11, 1974 in Quezon
City to spouses BELLA MANUZON BANDALES and MARIVELO DIVINA
ONTOG both deceased, for having been neighbors and friends for a period of time;

2. That in her POLICY her first name was only entered as MARIE CURIE B. ONTOG
which are different from MARI CURIE B. ONTOG the true and lawful one;

3. That we hereby certify that the aforesaid first name is refers to one and the same person
and no other one;

4. That we execute this affidavit to request the proper authorities to correct her first name
from MARIE CURIE B. ONTOG to MARI CURIE B. ONTOG;

5. That we execute this affidavit to attest to the truth of the foregoing facts and
circumstances, and to hereby voluntarily and knowingly that the true and correct name
which is only a typographical error/misrepresentation, and finally, we executed the same
for all legal intents and purposes.
