AFFIDAVIT OF UNDERTAKING
I, MARK HENDRO T. ESPINOSA, of legal age, single, and a resident of Hibbard Ave.,
Tubod, Looc, Dumaguete City, after having been sworn to in accordance with law, do hereby
depose and state, THAT:

1. I am currently applying for a Business Permit for our store with the business name
MARFELIA’S REFRESHMENT, located at Looc, Dumaguete City, with the Office of the
City Mayor of Dumaguete City;

2. One of the requirements for such Permit is for me or any of my representatives to attend
a Food Handlers’ Course conducted by the City Health Office of Dumaguete City.
Unfortunately, as of this writing, we have not attended such Seminar because of the
constant unavailability of slots;

3. I hereby undertake to attend or cause any of my representatives to attend a Food

Handlers’ Course by March, 2020 as required by the City Mayor’s Office;

4. I am executing this Affidavit to attest to the truth of the foregoing, and for whatever legal
purposes this may serve best.
