TELCO RENTAL COMMISSION AGREEMENT

The present Telco Rental Commission Agreement is concluded BETWEEN _______________

(Land Owner/ First Party) Filipino, of legal age and residing at _____________________ and
___________________________ (Independent Agent/Second Party) acting as a mediator.

UNDERTAKING

THAT, the agent will assist the Land Owner from gathering the requirements to the processing
of application for Telco rental for 30 years duration with the certain parcel of land ;

THAT, the Lot owner is obliged to notify the Agent in case of unexpected inspection of a DITO
Representatives and inform them about the discussions regarding the rental rate;

THAT, the NET AMOUNT to be received by the land owner He/she shall hand over the ___% to
the undersigned agent;

NOW, THEREFORE, in the event of the First Party to submit an application to other parties
and/or agents, the undersigned agent have the right to ask the company for disapproval and
find a replacement within the area.
