AFFIDAVIT OF LOSS

I, ELEONOR L. DINLAYAN Filipino, of legal age, Midwife II at National

Commission on Indigenous People- Provincial Office, and a resident of Barangay Patin-ay,
Prosperidad, Agusan Del Sur , after having been sworn in accordance with law, hereby
depose and state THAT:

1. The OMAOBAU CLAN Clan Organization, a Manobo ICCs/IPs of San Francisco

Agusan del Sur applied for Certificate of Ancestral Domain Title (CADT) for
formal recognition of their claim over their ancestral domain.

2. The NCIP-Agusan del Sur Provincial Office assigned me as the focal person for
the delineation and titling of ancestral domain of the Manobo ICCs/IPs of San
Francisco Agusan del Sur;

3. As part of the process, the NCIP surveyed the areas identified by the ICCs/IPs as
their ancestral domain, and consequently come up with technical description.

4. In September 2019, the ancestral domain’s technical description was posted in

every barangays covered, and as proof of such posting I secured a certificate of
posting from the barangay officials.

5. After posting the technical description, I took custody of Certificates of Posting as well as the
Receiving copy of Technical Description signed by barangay officials.

6. However, sometime in January 2020 I made an inventory of CADT documents and I can no
longer find the Certificates of Posting as well as the Receiving copy of Technical Description.

7. I exercised due diligence in locating the said documents and reported its loss to our NCIP
Provincial Officer, Patin-ay, Prosperidad, Agusan del Sur;

8. My efforts to locate Certificates of Posting as well as the Receiving copy of Technical

Description failed, until now the same has not been found, leaving me with no other recourse but
to request for reposting of technical descriptions.

9. I attest to the truth of the foregoing statements, and for whatever purpose this may serve.
