JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, SILVESTRA P. MONATO and REBECCA A. LAUREL, both of legal

age, Filipinos, and presently residing at Purok 13, Brgy. Poblacion, Prosperidad,
Agusan del Sur after having been duly sworn to in accordance with law, depose and
state:

1. That we personally know ARTHURO CHUA, residing at Purok 13, Poblacion,

Prosperidad, Agusan del Sur and died on July 3, 2020,
2. His children Daniel Chua and Jhon Chua were our students in the Monato
Institute of Technolgy.
3. That the birth certificate of his children were submitted when they enroll, and the
name of their father ARTHURO CHUA was reflected in our records.
4. We were surprised that in the marriage contract it shows Arturo Chu instead of
Arthuro Chua.
5. We believe that there is a clerical error in the name threin and ARTURO CHU
and ARTHURO CHUA refers to one and the same person.
6. That we are not his relatives either by affinity or consanguinity
7. Hence, we are executing this Affidavit to attest to the foregoing facts and for all
legal intents and purposes.
