AFFIDAVIT OF TWO DIS-INTERESTED PERSONS

We, ROLANDO P. LEON, of legal age, Filipino Citizen, with residence and
postal address at 090 Proper Alapang, La Trinidad, Benguet, and LOURDES
MARGARET A. SANTOS, of legal age, Filipino Citizen, with residence and
postal address at No. 080 Proper Alapang, La Trinidad, Benguet, after having
been duly sworn to in accordance with law hereby depose and state under
oath, that:

1. We personally know JEROME CHIOK, a consumer/member of the

Benguet Electric Cooperative (BENECO), said person died sometime in
2005;

2. Under the name of the said person are Two (2) BENECO Accounts
connected to two (2) residential houses, as follows:

A. Account No. 5190-1260-02-09; and

B. Account No. 5190-1290-02-6.

3. In the aforesaid accounts, however, the said person has different names
written, in Account No. 5190-1260-02-09 the name written is JEROME
CHIOK, while in Account No. 5190-1290-02-6 it is EUGENE CHIOK;

4. The correct first name of the said person is “JEROME” and same name
we known and also same name he’s been using during his lifetime and
never did use “EUGENE”. It only happen that during the processing of
the BENECO connection under Account No. 5190-1290-02-6, the name
“EUGENE” was inadvertently written instead of “JEROME”;

5. During the lifetime of the said person, he did not bother to correct his
name because what was important for him was that he’s been paying
regularly the electric bill;

6. With the foregoing circumstances, the name “JEROME CHIOK” as per

Account No. 5190-1260-02-9 and “EUGENE CHIOK” as per Account No.
5190-1290-02-6, pertains to one and the same person;

7. We are not in any way related to the above-mentioned person neither by

consanguinity nor affinity;

8. We know the above facts and circumstances because we are long time
friends and neighbors of the aforementioned person;

9. We are executing this affidavit to attest to the truth of our foregoing

statements to support the transfer of the two (2) BENECO Accounts to
Naty Tereng Chiok, and for all legal intents and purposes it may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 1st day of
March, 2023 at Baguio City, Philippines.
