AFFIDAVIT OF NON-FILING
OF INCOME TAX RETURN

We, spouses, SANTIAGO BELMONTE BALANCE and LORNA BRON

BALANCE, Filipino citizens, of legal age, and presently residing at B8 L17
Sinigwelas St., Golden Acres, Talon Uno, Las Piñas City, after duly sworn to in
accordance with law, depose and say:

That we are the biological parents of SHEILA MAE BRON BALANCE;

That SHEILA MAE BRON BALANCE is incoming First Year Student at

Cavite State University, Tanza Campus, Tanza Cavite;

That our said daughter is applying for scholarship program under

Megaworld Foundation;

That one of the requirements is a proof of the Bureau of Internal Revenue

(BIR) validated Income Tax Return (ITR) of parents;

That we did not file out ITR at the BIR because our income does not exceed
Php. 60, 000.00 in Maintenance Engineering and Housekeeping in Newport City,
Pasay City, respectively;

That we executing this affidavit to inform the proper authority of the truth of
the foregoing facts for whatever legal purpose it may serve.
