AFFIDAVIT OF NON-FILING

I, MARIO LITO DANGEL, of legal age, Filipino, and a resident of Jesuit St. Lower Sta.
Ana, Brgy. Sun Valley, Parañaque City, after being sworn to in accordance with law, do hereby
depose and say the following:

1. That I am the registered owner of ANGELJHUN SALON with business address at Jesuit St.
Lower Sta. Ana, Brgy. Sun Valley, Parañaque City;

2. That the said business was established last December 2, 2019;

3. That I did not file my Quarterly VAT (255Q) and Monthly VAT (2551M) at the Bureau of
Internal Revenue (BIR);

4. That I am executing this affidavit to attest to the fact that the owner did not file his financial
statement for the reason stated and for all legal intents this may serve.
