AFFIDAVIT OF UNDERTAKING

I, ANGUS PAULO POLI BLAIR, of legal age, Filipino, and a

resident of No. Via De La Luna St., Miraleste Grove, Merville,
Parañaque City, after having been duly sworn to in accordance with
law, depose and say:

1. That I am the registered owner of business name

VIETNAMIT RESTAURANT which is located at Sta. Ana
Drive, Sun Valley, Parañaque City;

2. That I undertake to submit LESSOR’S PERMIT after two

(2) months;

3. That I understand that should of VIETNAMIT

RESTAURANT fail to comply with this undertaking its
Business Permit may be REVOKED;

4. That this undertaking is executed to guarantee

compliance with all of the above facts and for whatever
legal intents and purposes this may serve.
