AFFIDAVIT OF LOSS

I, EMELIE G. WONG, of legal age, Filipino, and with residence address at

Block 7-A Lot 1 Genesis Ave., Juana 6, San Francisco, Binan City, Laguna,
after having been duly sworn in accordance with law, do hereby depose and
say, that:

1) I was the proprietor/owner of a business establishment undet the

name and style “WONG’S FASHION BOUTIQUE” located at Pacita Commercial
Center, San Pedro City, Laguna;

2) In January 2022 I closed shop due to bankruptcy as a result of the

Covid-19 pandemic.

3) In view of the confusion incident to the closure, I seem to have

misplaced the necessary documents pertaining to said business, particularly
its Business Permit and related papers.

4) I execute this Affidavit to attest to the truth of the foregoing narration,

and for whatever lawful purpose it may serve.
