I, JOSE G. TARAYA of legal age, Filipino citizen, and a resident of Lot 10 Block 5, San
Isidro Village, Virac, Catanduanes, after having been duly sworn in accordance with law, hereby
dispose and say:
1. That I was employed at the DILG with the position of Local Government Operations
Officer (LGOO) VI.
2. That I am transferred to the office of LGU Virac, Catanduanes Effective July 1, 2022.
3. That I hereby authorize DILG to DEDUCT from my salary/ retirement benefits all my
financial obligations/ payables with employer.
4. That I have executed this affidavit to attest to the above mentioned statements, for
whatever purpose this may serve.

1. That I was employed at the DILG with the position of Local Government Operations
Officer (LGOO) VI.

3. That I have NO pending criminal case nor administrative case.
