1. That I am the registered owner of two (2) parcels of land covered by Transfer Certificate of
Title Nos. 146-2018014188 and 146-2019002688 located at Lot 9, Blk 6, Villa Mercedita,
Brgy. Dumoy and Lot 10, Blk 6, Villa Mercedita, Brgy. Dumoy, Davao City, respectively;

2. That my daughter ADELAINE FAITH ZERNA-LEI and her husband JAN MICHAELSON
A. LEI are desirous to construct a residential building over the said parcels of land at her
own expense;

3. That I hereby consent to the construction of a residential building over the said parcels of
land by my daughter and son-in-law;

4. That I hereby further waive any liabilities and/or responsibilities on the construction of the
said residential building and further states that the same is owned by my daughter and son-in-
law;

5. As such I executing this affidavit in order to attest to the truth of the foregoing facts and for
