AFFIDAVIT OF CONSENT

 MEMORANDUM OF AGREEMENT
 
 
KNOW ALL MEN BY THESE PRESENTS:
 
 
This Memorandum of Agreement entered into by and between-
 _______________________, of legal age, Filipino, and resident
of _____________________________, hereinafter referred to as the “SUB-
CONTRUCTOR”;

BRLL Electronics Network Solutions, represented by
__________________________________, likewise of legal age Filipino and resident of
_____________________________________________, hereinafter referred to as the
“SECOND PARTY”
 
 
WITNESSETH, That-
  
WHEREAS, the First Party/Sub-Contructor is/are the lawful and absolute
Representative of __________________________ located
at________________________________.

WHEREAS, the Second Party are the duly constituted agents by BRLL
Electronics Network Solutions to assist the lot owner in gathering lease requirements for
DITO TELECOMMUNITY, the new telecommunication corporation sanctioned by the
proper authorities to operate in the country;
 
WHEREAS, the said DITO TELECOMMUNITY is now in the process of building,
among others, its transmission tower/ cell site nationwide to provide effective
cellular/network service the Second Party being the duly constituted agents of BRLL
Electronics Network Solutions, are tasked to find potential sub-contractors for the
purpose of building and constructing its cell site, for a certain period of time.
 
 WHEREAS, should the Company offered for accreditation by the First Party, be
qualified and approved as Sub Contractor in building Cell sites;

WHEREAS, the first party will give 10% of the total amount per tower for
facilitation fee of the BRLL Electronics Network Solutions and agent’s commission.

WHEREAS, the scope of this agreement will cover the total number of projects
and duration of the project.
 
WHEREAS, the Second Party proposed to the First Party the latter’s participation
by submitting the company’s pertinent and relevant documents, for evaluation and
determination by the Company whether the said company is competent and capable for
the construction and erection of its transmission tower to which such offer, including its
terms, was accepted by the First Party, the company representative;
 
 
NOW, THEREFORE, for and in consideration of the foregoing, the parties hereby
execute this Memorandum of Agreement under the following-
 

TERMS AND CONDITIONS

1.  TERM OF THE AGREEMENT. This Agreement shall be covering the total

number of cell site and duration covered thereafter upon mutual consent by the
parties;
  
2. CONSIDERATION. The amount of the agreement for the duration of the
contract shall be paid by check or be deposited directly to the bank account of
the second party.
