Dear Sir:

It is with deep regret to inform you that I will be resigning from my position as
Administrative Assistant of this office for Davao del Sur because of heath reasons
effective thirty days from receipt hereof.

It has been my pleasure serving the government for almost a year but I believe that I
cannot effectively discharge duties because of my illness. My work as an
administrative assistant gave me confidence and new insights that I can use in any of
my future endeavors and work. I am hoping for your kind consideration.
