Oro, Philippines, Do hereby named, constituted, and appointed ZORINA MAY S. ALMADIN, Filipino, of legal age,
married with postal address at Purok 1, La Filipina, Tagum City, , Filipino, of legal age, married with postal address at
Purok 1, La Filipina, Tagum City and MARVEN B. PATCHO, Filipino, of legal age, single with postal address at Purok

2. Or to process, transact and follow-up regarding my Business Retirement, Change line of Business, Dropping Line
of Business, Change Address, Change RDO and Separation of Employee at
LGU/BIR/FPA/DTI/SSS/PHIC/HDMF

3. To execute, acknowledge, and sign all such receipts, contracts, deeds or other instruments as maybe necessary in
connection therewith;
4. To pay the required fees, in relation therewith;
5. To submit all other documentary requirements as may be necessary for the purpose of processing and other legal
purposes.
6. To accept, receive and sign all documents and papers in relation thereto;
7. To DO and PERFORM all acts and deeds which may be necessary or proper to carry the above purposes.

HEREBY GIVING AND GRANTING full power and authority unto said attorney-in-fact, to do and perform all
acts and deeds whatsoever requisite or necessary to be done in and out the premises as full authority to all intents and
purposes.

HEREBY RATIFYING AND CONFIRMING all that said attorney-in-fact shall lawfully do or cause to be
done by virtue of these presents.
