AFFIDAVIT OF WAIVER

I, MERLYN P. FAMA of legal age, Filipino and a resident of Brgy. Brgy. Sambag, Jaro,
Iloilo City, Philippines, after having been duly sworn to in accordance with law, depose and state:

1. That I am the Quarry Permittee of Ordinary Earth Quarry with Permit No. QP-17-005 (Ma),
situated at Brgy. Nasaka, Maasin, Iloilo;

2. That out of the said Quarry Permit, I deposited/lodged the amount of TWENTY
THOUSAND PESOS (P20, 000.00) as Monitoring Trust Fund (MTF) and the amount of
SIXTY THOUSAND PESOS (P60, 000.00) as Rehabilitation Cash Fund (RCF) at LAND
BANK OF THE PHILIPPUNES-Plaza Libertad Branch, Iloilo City on August 10, 2017;

3. That the said Quarry Permit has expired on October 14, 2022 and the Land
Owner/Administrator Mr. Ricardo M. Morales desires to renew the said Quarry Permit
under his name;

4. That I WAIVED the amount of TWENTY THOUSAND PESOS (P20, 000.00) as Monitoring
Trust Fund (MTF) and the amount of SIXTY THOUSAND PESOS (P60, 000.00) as
Rehabilitation Cash Fund (RCF) in favor of the Land Owner/Administrator Mr. Ricardo M.
Morales to be used in his application for renewal of the above-mentioned Quarry Permit;

5. That I am executing this affidavit to attest to the truth of the foregoing facts and for whatever
legal intents and purposes that it may legally serve.
