AFFIDAVIT OF ONE AND THE SAME PERSON

I, JOEL MARIN, JR., of legal age, married, Filipino and a resident of Bingauan, Iloilo,
Philippines, after having been duly sworn to in accordance with law, depose and state:

1. That I am a Child of Joel Marin, Sr., who died on May 3, 2021 at Janiuay, Iloilo;

2. That my father’s name in his Death Certificate appears as Joel Marin Sr.;

3. That we are processing our father’s death claim at Social Security System (SSS), but his
name in SSS’s records appears as Joel Marin;

4. That the name Joel Marin that appears in the Death Certificate and the name Joel Marin that
appears in SSS’s records refer to one and the same person;

5. That I am executing this affidavit to attest to the veracity of the foregoing statement to prove
that Joel Marin, Sr., and Joel Marin are one and the same person and for whatever legal
purpose it may serve.
