I, JOANN BALISTA JULIAGA, of legal age, married to NELSON A. JULIAGA, Filipino

and a resident of Brgy. Dangula-an, Anilao, Iloilo, Philippines , after having been duly sworn to in
accordance with law, depose and state:

1. That I am known by the name JOANN BALISTA JULIAGA and consistently used the
same in my personal and official records and interactions;

2. That my name was entered in my Certificate of Livebirth issued by Philippine Statistic

Authority (PSA) as JOANINA DIDAL BALISTA;

3. That the name JOANN BALISTA JULIAGA that I consistently used in my personal and
official records and interactions and the name JOANINA DIDAL BALISTA that appears in
my Certificate of Livebirth issued by Philippine Statistic Authority (PSA) refer to one and the
same person

4. That I am executing this affidavit to attest to the veracity of the foregoing statement to prove
that JOANN BALISTA JULIAGA and JOANINA DIDAL BALISTA are one and the
same person and for whatever legal purpose it may serve.
