2018000068 located at Brgy. Maorigao, Caraga, Davao Oriental;

3. That as an heir of the above-mentioned parcel of land, I am not voluntarily GIVE, TRANSFER
and CONVEY my share of rights and interest to a certain __________________ a resident of

4. That I am not belong to the other siblings of my deceased mother in regards to conveying their
